Athletic Advantage Physical Therapy is in-network with Blue Cross Blue Shield and Medicare. Have questions about your coverage or benefits? Give us a call and our team will be happy to help.

Self-Pay:

We also offer self-pay options for patients who do not have insurance or prefer not to use their insurance for physical therapy services. Contact us to learn more about self-pay options and pricing.

Cost of Service:

Initial Appointment – $275

Additional Appointments – $175

​​What Should I Bring to My First Appointment?

• Fill out the patient intake forms that will be emailed to you once your appointment is confirmed. Please be sure to include your insurance card and Photo ID (we require a copy for our records).

• A prescription for physical therapy from your referring Physician, ARNP or PA. You can also have your doctor fax your prescription to us at (800) 213-4413.

• Please arrive 10 minutes early to complete any remaining forms, so you can maximize the time in your evaluation.
