If you are a first-time patient at Athletic Advantage Physical Therapy, don’t worry! We will do everything in our power to make you feel relaxed and confident. Here is our mission statement, and some basic information to help you familiarize yourself with our Allen physical therapy office.

At Athletic Advantage Physical Therapy, we want to maximize your time with us. Prior to your first visit, please print and complete the New Patient Forms below. Please bring these completed forms, your insurance cards, and photo ID with you to your first visit. Otherwise, allow extra time before your appointment to fill them out here.

What to Expect on Your First Visit
Being well prepared for your appointment will ensure that we have all of the needed information to provide the best possible care for you. You may want to write down a history of events related to your condition before you arrive. This may help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. We look forward to your first visit.
Wear loose, comfortable clothing and appropriate footwear
Arrive a few minutes early to complete and review new patient registration forms
Meet with a physical therapist for a thorough initial examination and assessment of injury or condition
Review goals and specific needs to develop an individualized treatment plan
Determine the frequency and duration of your course of physical therapy based on rehabilitation objectives
Discuss patient expectations for maximizing therapy visits and home exercises. Active participation from the patient is vital to a successful recovery and rehabilitation experience
